In consideration for being allowed to participate in a hot air ballooning event, and with full knowledge that ballooning can be a dangerous sport with risk of personal injury including loss of limb or life, I, _______________________________, on behalf of myself and my heirs and assigns, (1) certify that I have no physical or mental condition that would impair my ability to withstand the rigors of the sport, (2) voluntarily assume any and all risks inherent in the event, and (3) release and discharge Bear Balloon Corporation, Richard Behr and all persons participating in the event at his direction from any and all claims and liabilities, of whatever kind of nature, including claims for personal injury or property damage resulting from my participation in this event.
